Access Anything is a website for an group based in Steamboat Springs, Colorado, that offers travel advice with emphasis on outdoor activities.
Accessible Travel Worldwide is a rich site with links to accessibility in many parts of the world.
Accessible Nature Travel: A travel guide for folks with wheelchairs, walkers, canes or strollers by Randall Feare is a travel guide to 22 states and British Columbia. There is also a list of valuable links to additional resources.
Dignity Travel offers “Easy and Accessible” tours around the world. Kristin wrote that “I went on a guided tour to Iceland with… [them]…. and it was excellent. It was 100% wheelchair accessible and great for slow walkers. They even provided a wheelchair for me as I was a slow walker with endurance issues. I would recommend them to anyone looking to travel with a small group. They do trips all over the world that change from year to year but also have yearly trips to Machu Picchu and the Galapagos Islands.” Here is their website.

TabiFolk formerly called “Accessible Travel Forum” is a rich source of information sharing for disabled travelers and those serving them. “It is more than just a forum, it is a place for conversations. You can share great information you found, or ask for help from a local resident or a fellow traveler who went before you, you can even find a shoulder to cry on if you had a bad experience.” Thank you Josh for letting me know about this resource.
The Disabled Traveler’s Companion by Ron and Kay Wilmer, with great photography by their son, Roger Wilmer. This excellent website gives information about five of the National Parks and Tahquamenon Falls State Park in Michigan.
The Handicapped Travel Club “… was formed in 1973 to encourage traveling for people with a wide range of disabilities. The HTC encourages people with disabilities and their families to travel, to meet and to share information on making recreational vehicles accessible for the disabled.”
